Long-Term Guest Authorization Form

This form is intended to allow residents to authorize any relatives (not living in Lee or Collier County) to receive a guest pass for more than 30 days but not to exceed 90 days. It is not intended to replace the rental application for lessees. Extensions of these passes require special permission; please call the office at (239) 947-5977 for more information.
